1pw version 3 after version 5 trial

I have been using 1PW version 3 and then tried version 5 on Mac OS X Yosemite. After trial period and declining to purchase version 5 I now can not use CMD / shortcut in Safari to bring up 1PW pop up window and can only bring up 1PW by clicking on a 1PW tool bar icon. Also there are 2 of these tool bar icons in Safari's tool bar. Deleting and reinstalling the 1PW safari extension 3.9.20 results in the same problem reappearing.
Also I see there is a red exclamation point in the 1PW popup dialog.

    Hi @rlynch‌

    Can I just confirm, are you wanting to remove 1Password 5 completely and revert to using just 1Password 3? If this is the case can I ask what steps you've taken so far.

    Once we have a better idea we can assist.
