Odd behaviour ?!
Saving a password in Firefox 34.0.5 (Mac OSX 10.9.5) save the password with another URL (from another tab) !
I can't anymore trust 1password to store and retrieve passwords.
Often the Username is not stored with the password or the password is stored in the Username field. Last but not least, where and how are the Data crypted ? Do you know some bad software able to modify ( change) the keyfile on the cloud ? To be honest I'm very disappointed with so many problems.

Can you walk us through how to replicate this please. By this I mean a step by step guide. If we can replicate the bug we can work on sorting it out.

First- I should mention I managed the odd items by editing manually them and updating into 1Password via Firefox extension whatever you call it.
- I guess 1password is probably not the culprit ! I think it may be my previous Time Machine restoration that cause this mess.
1/ I create an account log/pass on a site using the 1password tool.
2/ I save it into 1password
3/ generally a Mail is sent by the site to confirm; I click on the link and have access to that site.When I want to log in again ( another day, or session) I can't. Because either the Name, pseudo, mail, ID was not saved at point 2/ ( I have only the password field filled)
Or the record in 1password is refering to two different sites ! not related. I know it's very odd.

That is correct. Put simply you need to be running OS X Yosemite to use iCloud to sync with your phone. All the details are here in this knowledge base article (because it's slightly more complicated than my simple confirmation).

@Stephen_C I finally give up ( for today ). You should have hired a translator. Things are really complex. I have 2 different OS X versions, 3 1password databases ( one for each device), no simple way to sync. How other customers without any knowledge of English do ?

1/ I create an account log/pass on a site using the 1password tool.
Do you happen to mean you use the Password Generator to create a password? (see the image below).
If you use the Password Generator to create a password it will create a Password item. All that item will contain will be the name of the site as the title and the password. A Password item is very separate from a Login item. When you log into a site for the first time you should get a small window pop up that looks like this:
That will create a Login item and that should be storing the username, password and the website URL (and some other stuff too). If you create a Login item that has the same password as a Password item the Password item will be converted, otherwise Password items are kept as a safety measure.
Does that in any way relate to your issues over the username field?

In regards to your syncing problems.
If you are running a mixture of 1Password 3-5 then your best option is Dropbox. 1Password for Mac, versions 3-5 and 1Password for iOS, versions 4-5 can synchronise using the agilekeychain that we use with Dropbox.
We can take you through the steps but before we proceed it would be prudent to understand what is in each of your vaults at the moment. For example, do you have unique items in each vault? Is it one or two items or do you have a lot of items that are present in only one of your three vaults?
You probably have the most complicated situation at the moment. Syncing is much easier if you're starting with a fresh copy of 1Password and simply want to sync an existing vault from another device. It gets messier when you have 3 existing vaults.
As I say, if you give us a better idea of what we're dealing with in terms of your 3 vaults and we can come up with a plan of what to do.
