Needing 3 Fields to Login to an account

I have 1Password with Pro features, trying to set up a login to one of my often used accounts but I need a member no. field, a surname field, then a password field. Can you help with this please?

If you use 1P on a Mac what you'd normally do to solve the issue is to re-create and save the login manually. Unfortunately I don't believe you can do that on an iOS device. Do you by any chance have access to 1P for Mac, which you could use and then sync the data to your iOS device?

Thank you for your response. Unfortunately I don't have a Mac only a PC.

I've never used 1P for Windows but from a look at the knowledge base it seems you can do the same thing with 1P for Windows. I strongly recommend trying that.

Thanks for the answer, @Stephen_C, you should be able to do it on the desktop. That's the best way to solve the issue.
