Because each platform requires a separate license, yes, you will also need to buy a Mac license separately from your iOS license.
You won't need to reenter all of your data, though. 1Password provides for synchronizing your data by one of several methods. The cloud based ones are Dropbox and iCloud (but iCloud only if you buy your Mac license from Apple at the Mac App Store). There are also provisions for syncing by WiFi or Folder sync, for those who don't want a cloud-based solution.

@dperrijr, the information from hawkmoth is spot on, and I just wanted to reiterate that 1Password for Mac is available from our web store, and from the Mac App Store. The main difference is that only the Mac App Store version has the iCloud Sync option. (Both the web store and Mac App Store versions have Dropbox, Wi-Fi, and Folder sync.)
If you're currently syncing your 1Password data between your iPhone and iPad via iCloud or Dropbox sync, it should be very easy to sync that to 1Password on your Mac. Basically, just choose "I've used 1Password before" during the setup, then choose the same sync method.
