Fill-in forms not working on iphone5.
I have 1PW loaded on iPhone6. (Don't know how to find the version number.)
I have synced with computer (don't recall if it was iCloud or other method and don't know how to tell). My passwords are there, but web forms do not autofill w username and password, as it does no computer. What am i doing wrong?

@dcampell you have posted in the wrong forum. I am moving your post to the 1Password for iOS forum where you will get more help.
You can find the version number by going to 1P Settings and looking towards the bottom of the screen. But before we can help more you must tell us how you sync and what you sync with. Are you syncing with a Mac or with a PC? What version of 1P do you use on the Mac or PC? (Go to 1P > About 1Password and tell us exactly what it says there)?

Thanks for the advice. This is first time posting on this forum.
I am syncing an iPhone 5, running IOS 8.1.3 to a Mac running OSX 10.9.5. I am NOT using Dropbox for syncing, so I presume I am using iCloud (not sure how to tell this).
Question: on the iPhone, when a web form opens and asks for password info that is in 1PW, should it automatically fill in, or do i have to take some action to make it do that?

I hope you don't mind if I refer you to a knowledge base article. If you take a look towards the bottom of this knowledge base article there are links to other articles explaining how to login to sites using Safari, how to login to supported apps and how to login to sites using the in-built 1Browser. I think you'll find those helpful.
I'm still a little mystified about how you sync your 1P data. You can't use iCloud if you're using iOS 8 on the phone and OS X 10.9.5 on the Mac: that isn't going to work. If on the phone you go to Settings > Sync what does it say there under Sync Service?

I will look at the article. Thank you for the reference. As far as syncing, I go to Settings on my phone but I don't see anything labelled Sync Service? Is there another way to get to it?

So far as I can see (it's rather early in the morning here!) you've still not told us what version of 1P you are using on your iPhone. Please go to the 1P Settings screen on the phone, look towards the bottom and tell us what version of 1P you have.
When you click on the Settings button in 1P on your phone I'm expecting you to see this screen shown in this knowledge base article. Do you?
If not, is it possible, please, to post here a screen shot of the screen you do see?

I found the 1PW version number for iPhone app: 5.2.1
I do not see the knowledge base article you referred to when I am in Settings in 1PW on the phone. I made a screenshot of the phone showing what i did see, and saved it as PDF on my computer, but this discussion site would not let me post the PDF to this message.
1) When I try to open a website which is listed in 1PW, the screen for Username and PW appear, but 1PW does not fill it in automatically. Isn't it supposed to?
2) Apparently not syncing: I learned that the phone offers sync by Wi-Fi, which is what i chose. However, when i open the app on the computer, it only offers two choices: Dropbox or iCloud (which requires ver 1PW5). Not WiFi. I have 1PW4 on the computer.

Only if you open the site from within 1Password. If you're in Safari you'll need to invoke the Safari extension to have 1Password fill:
