Yes—custom fields in secure notes are not yet supported by 1P for Mac.

They are in the current 1P for Mac beta version which is currently under test. Before you ask :) I don't know when it will be released—and AgileBits won't tell you that either. But maybe the news is some encouragement for you.

Ah, That's why I could see the feature on my Home machine and not here as I have just done a fresh reinstall of Yosemite and haven't switched to the beta channel. Will do this now.
Also, I wasn't intending to ask when it was going to be released so please don't be like that. I realise that software is done when it is done.

I'm glad Stephen's answer was helpful for you! As he mentioned, support for custom fields in Secure Notes was added in a recent beta version of 1Password for Mac, so it should be included in an update for the release version sometime in the near future.
