Using the password generator button when editing an item
next to the password is the 'combination wheel icon' which when clicked appears to replace the old password with a new, randomly generated password. i've lost many passwords by accidentally clicking the combo wheel icon. shouldn't there be a confirmation dialog before erasing an existing password? this behavior seems so dangerous I feel I must be doing something wrong; but, being a typical user, the program shouldn't allow me to casually erase passwords and destroy my access to my files. I'd like a fix or an explanation, thanks.

I'm sorry for any confusion about that! The 'combination wheel icon' is the button for our password generator. You would only want to click on that button if you want to generate a new password to replace the current one in the password field.

Can you elaborate on what you're trying to do when you click that button by accident? That button only appears when you're editing an item. If you're trying to make an change to the existing password, you can click in the password field itself, instead of the button next to it.
Or, are your passwords normally concealed as black dots, and you're trying to see the actual password? If so, you don't need to edit the item at all. Simply hold down the Option key on your keyboard to temporarily see the passwords. If you want to always see your passwords instead of the dots, go to the View menu and disable the option for Conceal Passwords.

The password generator button is only available once you click the Edit button for an item, and your changes won't be saved unless/until you click the Save button. If you don't want to save the changes, click the Cancel button, then click Don't Save.

Again, in order to actually change your password this way, you would need to click the Edit button for an item (indicating you intend to make a change), then click the password generator button next to the password field, then click the Save button (indicating you want to save your changes, including the new password).
But even if you do that, your old password is not lost. If you've changed a password (and saved those changes), that item will have a
show previously used passwords
button. Click that, and you'll see your previous password(s) for that item. If you changed a password by accident, copy the old one from there, then edit the item and paste it in the password field - and remember to save the changes, of course. ;)I hope that helps to clear up the confusion!
