Extension not showing in Safari (or anywhere)
Running iOS 8.4 on an iPad Air 2, latest version of 1Password (pro version).
I've followed the instructions on how to enable it (having opened 1Password, restarting the computer etc).
I click on the Share "button" > More, and the sheet just shows "Message, Mail, Twitter, Facebook, Gmail" only.
How do I get the extension to show?
1Password Version: 5.01
Extension Version: Not Provided
OS Version: Not Provided
Sync Type: dropbox

You need to activate the extension. At the end of the list of actions, below the list of application you mentioned, mtheir is an icon with three dots. Tap that, and you'll find a list of other things you can activate, including the extension for 1Password. Turn that on. You can also control where in the list the extension appears.
I'm not sure what you mean by "or anywhere." 1Password will work with iOS applications (not web pages) only if the software developer includes code that allows the app to communicate with it. If the developer doesn't do that, 1Password can't fill credentials because of iOS sandboxing.

To follow up, here are some screen captures to illustrate turning on the 1Password extension.
Tap the More option in the second row:
Turn on the entry for the 1Password extension:
The newly activated 1Password extension appears in the list in the same position as the previous screen:
