1P ver 6 not working in Safari in iOS 9
When I tried to use 1P to generate a login, 1P at first only showed the identities and credit card lists, but nothing else. I exited 1P back to Safari, and tried to access 1P again from the same web page. Even though only a few seconds had passed, 1P provided the login screen again, but this time no matter what I did it would not bring up a keyboard.
Just now (several hours later) I first opened the 1P app itself (so I could check on the version number, a VERY difficult thing to find), then I returned to Safari and brought up 1P. It was still presenting me with the 1P login screen with no keyboard.
Between the 2 sessions I had rebooted my iPhone, so I don't think this problem is going away.

Is this what you were seeing?
If so, does the keyboard appear when you tap into the Master Password field?
Also, are you using third party keyboards?

Your picture is what I see. When I tap, the keyboard does not appear. I am not using any 3rd party keyboards.
I did the following troubleshooting. I copied my 1P password into a notepad app on my iPhone. From Safari, I was able to paste it into the 1P helper (above pic) after tapping. 1P opened, but my URL wasn't there and so I tapped on the search field but, again, got no keyboard.
If I open 1P directly (not from Safari) on my iPhone, I do not have this problem.

@budsimrin: Unfortunately we rely on iOS to offer the keyboard, so there aren't a ton of options here. You mentioned restarting your device, and I'm sorry that that did not help.
But I see that you're using Dropbox. Do you have all of your 1Password data stored somewhere else safe? Since it sounds like only the 1Password extension is exhibiting this problem, the app may simply be damaged. Please backup your 1Password data, delete 1Password from your iPhone, restart it, and then reinstall a fresh copy from the App Store. If 1Password itself is the problem, that will resolve the issue. However, if you're still having trouble limited to only the extension, it may be a deeper iOS issue requiring more extreme measures...but we'll cross that bridge when we get to it. Please let me know how it turns out! :)

I tried something else first. I used the 1P Safari extension on my wife's iPhone 5 and was successful logging in to Facebook. I then tried the web page I am having problems with on my iPhone 5 (fuelly.com) and her phone exhibited the same flakey behavior as mine. Back to my iPhone, I tried Facebook and had no problem and then tried fuelly.com and got the bad behavior again. I then tried the American Express site and 1P worked fine there, too.
So, it seems the problem is associated only with that one web site. Maybe you can try fuelly.com and see what happens?

@budsimrin: Absolutely! Unfortunately I wasn't able to reproduce the issue myself, but I don't have an iPhone 5 to test with to see if it's device-specific (it sounds like it may be). Also, let me know the exact URL just to make sure we're testing the same thing:

I tried manually locking and force quitting 1Password, and made sure I didn't have content blockers enabled in case this was some weird scripting issue. But honestly, I can't imagine how the website itself could interact with the extension or iOS keyboard, so there must be something else going on here.
I will, however, say that I made a trip to the Apple Store earlier today concerning a battery issue, and they ended up resetting my device...and now I worry that may have some impact on testing. :(
I'm trying to see if someone here has an actual iPhone 5 (not just a simulator) to test this on, to see if it helps us reproduce the issue. In the mean time, I'm really glad to hear that other sites are working for you, but we'll see if we can solve this mystery! :)

Actually, the URL I used is only fuelly.com. On this web page is a place for User Name, Password, and a Log in button.
On my Mac, when I type this it sends me to www.fuelly.com, but in Safari on my iPhone it is simply fuelly.com.

Unfortunately, I tried this with the iPhone 5 simulator and my iPhone 5S, it is not happening for me either.
What about changing to use the PIN code instead of the master password, does that let you use the extension in Safari as expected?

Success! I enabled the 1P PIN code, and tried again. This time the web page behaved properly in Safari and the 1P extension provided the login for me. Now, it so happens that 1P on the iPhone 5 was still open, but let's call this a success and move on.

Excellent! Thanks for the update. On behalf of MikeT, you're most welcome! I'm glad that worked for you. It sounds like there may have been some issue with the iOS Keychain, since resetting the PIN would clear that. It sounds like you should be all set, but be sure to reach out if you need anything else! :)
