Could an automatic upgrade cause this to happen. I may have allowed an update to install, and this may be why I am on Version 5. I believe we purchased Version 3.

@MarcGuest - The upgrade from version 3 to version 4 required an upgrade payment, which also covers version 5. If you have a licensed copy of version 3, and you never paid for an upgrade to 4 or 5, that could indeed be the reason you are now seeing your installation as unlicensed. You would have a 30 day free trial, but after that, those reminders should set in.
Version 3 has not seen any development since version 4 arrived, and there have been a number of OS X updates since then. The latest update to OS X 10.11.2 has caused a number of users trouble with continuing to use version 3.

@MarcGuest: To be clear, 1Password has no way of knowing whether or not you've purchased it unless you register the app with your license information.
Now, if you've already purchased 1Password 4 or 5, there's no need to purchase it again.
