Got New iPhone, cannot open my 1password account on my iPhone after restoring from backup

I need help. I have 1 password on my Mac book air, my iphone and Ipad. I had to have my iphone 6 replaced. I backed up before getting new phone and than restored from that back up. When I try to open 1password on my new phone it asks me to sign in to account, when I click on that it has two choices 1. Scan QR code or 2. Sign in manually. Do not know where my QR code is and wnen I try manual sign on it asks for Team URL,Account Key, Email and Master password. I know email and master password but not the other two. I need to get my 1password functional on my new iphone. It works on Mac book and ipad but not my new phone. Can you help me please?

    @Matlas: It sounds like you don't have a 1Password Account. You only would if you'd signed up for 1Password Families (or Teams). That's the only way you could "login" to use 1Password. Otherwise you'll need to sync or copy your 1Password data over from another device, or restore from a backup:
