Shared vault (dropbox): password is missing from 1 of 3 devices (all Mac/iOS)

My partner (A) and I (K) use a single Vault (Dropbox sync) across three devices:

  1. [K] Macbook Pro (10.11.5) - 1Password 6 - Version 6.3 (630032)
  2. [K] iPhone 6s (9.3.2) - 1Password iOS - Version 6.4.2 (642004)
  3. [A] Macbook Air (10.9.5) - 1Password 4 - Version 4.4.3 (4430000)

It's been working out brilliantly until recently. There is a single login (added on Tuesday 6/28/16) that is showing up on Devices #2[K] and #3[A] but NOT #1[K].

When I check the status of Dropbox sync on any one of the devices, it claims to be up to date, and other logins added since then appear on all three devices. Any ideas?

P.S. We both use 1Password extensions across multiple browsers on our Macbooks, but since the problem seems to be with the "mothership" on Device #1[K] and since there are so many extensions, I figured I wouldn't list them unless absolutely necessary. Let me know if you need them.
