@JoeAldrich,

There should be a window in the upper right corner of the screen that displays this code and an acceptance button to confirm it. If you're using multiple Desktop Spaces using Apple's Mission Control features its definitely possible that this dialog isn't on the current Space so I would definitely recommend checking to see if the window is on another Space.
