Trying to use 1Password to save my login for SigmaCare, but can't save the "Account" field.

The login I'm trying to save is here -

I tried to save it as a new login as described at https://discussions.agilebits.com/discussion/85559/how-to-create-a-login-with-three-fields-username-last-name-and-password-1p-refuses-to-fill-it, but the Account field still isn't saved. Appreciate any help you can give me.
