Can't create 3-field login (for an AWS AIM login)

I'm trying to create a login for an AIM account on AWS.

This login uses 3 fields - Account ID, IAM user name and password.

I looked for other people with the same problem and followed sjk's instructions from this thread

However, even when creating a login entry manually, 1password still only save two fields (username and password) - not the 3 fields required for the login (and present on that page).

Hey @yahbai! What version of 1Password and the 1Password extension are you using? And what is the website you're saving the Login item on? We'll take things for a spin and see what the issue might be.
