Rich icons not correctly populating all panes

Some Rich Icons in 1P are not correctly populating. There are three basic panes in 1P, left, middle and right. The RI are Agilebits icons, not my own personal ones. The icons do appear to be in the system, the right pane is correctly populated. But the middle pane is either showing an old low resolution icon or in some cases nothing at all.

I have tried turning off RI and turning them back on again but it has no effect.

    Thanks for the report. Is this reproducible for you? i.e. if you save a new login item with the same website field as the existing item does the new item also show this behavior? If so can you share what URLs this happens with?

    @Ben Weird. Tried turning them off and on again and they have populated correctly. I'll keep an eye on it and report back if there are further problems.
