Profile shows access from device not used

On my profile, there was an entry from 10/10/18 stating that access was granted to a device (iPhone X iOS12) at 7:06 pm that has never been used to access the app. I promptly deactivated the device. Also the IP does not correspond with the device IP. Concerning? I have a screenshot to show details.

    OS version 12
    Correction: I had used the device to access 1Password once. I did not receive an email notification of this access. I did not perform this action with my iPhone at the time indicated. Advice?
