Download via Mac App Store or Website
I already have a 1Password for Families subscription purchased directly through your website. Can I login to the Mac App Store version of the client with my current account or would I need to create a new subscription through an in-app purchase?

You should be able to sign into your 1Password.com account thru the App Store version of the 1Password app. There would be no need to create a second account. Creating a second account would result in duplication of charges as well as duplication of services.

Indeed, while only the website version of 1Password for Mac works with standalone license purchases, both that and the Mac App Store version work with 1Password.com memberships: you just need to sign into your account when setting up the app and you'll be good to go. :)
