Welcome to the forum, @KeithEGould! If you're using 1Password in standalone mode, you'd add the attachment directly. Click "Edit" on your item, scroll down, and you'll see the attachments area, where you can drag in an image or other file which will be directly attached. If you've got a 1password.com account, it works a little bit differently: click "Edit," scroll down, then under the Related Items section, click "Add New File" - this will create a Document item, which you can use independently, but with a link to the original item you uploaded it under.
