This issue has been occurring in 1Pwd Mac only since March 2023, at least from 8.10.4 to 8.10.18, and is still occurring. Same issue on macOS 13 or macOS 14. I never had this issue on 1Pwd 7 or 8 Win...

I understand that 1Password is opening the wrong item after you unlock the app. I'm sorry for the impact that this is causing your workflow and I agree that this needs to be investigated further.
That being said, I haven't been able to reproduce this behaviour myself. Would you be willing to create a screen recording of yourself reproducing the issue? You can find instructions on how to create a screen recording here: How to record the screen on your Mac - Apple Support
