Why will my username and password not load on a particular website?

1Password TeamI'd hate for us to throw in the towel when this is, seemingly, a simple issue to solve. If you search for the item in your vault (from the desktop app), is it listed there? What vault is it saved to? If you go to Preferences > Vaults > Include in All Vaults, is the vault where the item is saved to selected, like this?
