1Password not working on Firefox 21 and OS X 10.6.8 [resolved in Firefox extension 3.9.16]

It appears that 1Password is not auto filling on Firefox 21. It seems to have just stopped. The icon has disappeared, too. No new add-ons. I tried to uninstall and re-install, but no joy.

Any suggestions?

What version of the 1Password extension do you have installed in Firefox?

What happens if you create a new Firefox profile and install ONLY the 1Password extension and nothing else?
