Wrong username for one website

I have just one website where 1Password offers to fill in an incorrect username. I've checked for other entries in 1Password for that site, but there is only the one entry. If I CMD-\ then it fills in the correct username.

Similar behavior on Windows 11!
I notice that both browsers (Win10, Win11) had 2 1Password extensions - the beta and the stable channel. I removed both and tried them one at a time, but no difference in behavior.

I'm sorry for the trouble you are having on this particular site. I have a couple of questions to try and better understand the behaviour you are running into.
If you go to the website and use the inline menu suggestions to fill your username instead of the keyboard shortcut, do things work as you'd expect?
Does the same incorrect username fill every time? If yes, can you locate that item in your 1Password and look to see if it has any linked apps listed within it?

The inline menu suggestions are what pop up after you have been validated? If so, then no. the only choice offered is the wrong choice. There are no linked apps for that entry. If I choose the "open and fill" option, then the correct username is entered.

@Stevew00t I think I'd be best for us to take a closer look at your setup as it sounds like the browser extension might not be syncing properly.
