Can't save a new login - clicking the popup save button does nothing?

So on Safari Version 18.2 (20620.1.16.11.8) on Mac Sequoia 15.2 when you use a new website the popup window appears with the new details and there is a save button - but clicking it does nothing.

1Password Version: 8.10.58
Extension Version: 8.10.56.28
OS Version: 15.2
Browser: Safari

    I'm sorry that you're unable to save your login Safari. I'd like to ask you to create a diagnostics report from 1Password in your browser:
