1Password won't save a 4th login (or 5th etc.) for the same site
I'm having an issue with 1Password 1.09.340 not saving a 4th login to BA.com
It also won't save any login for iCloud.com
I've tried it on both IE 11.09 and FF24
Please offer some guidance and thanks!

1Password does, indeed, have problems with the way the login "form" (it's not the usual kind of HTML form) at iCloud.com is coded. We're trying to figure out a way to handle it.

As for the ba.com question, I've used Firefox 25.0.1 to visit ba.com, entered three different username+password pairs, and not once did 1Password ask me whether I wanted to update the existing Login item for ba.com. Instead, I had the choice of creating a new Login item or replacing one of the existing Login items. I'll try to find out why that's happening.
I was able, though, to do that as many times as I liked. I stopped at six. Here they are in the main 1Password program:
