Safari Extension Not working [resolved; uninstalled v3, restarted Safari, installed v3]

I just tried to install the Safari Extension into Safari 6.1.1 and I am getting the error message "Safari can't open "onepassword4-extension://activate/Safari" because OS X doesn't recognize …….

What am I doing wrong? I am using 1P 3.8.21

    Hi @mikemoreau‌,

    For 1Password 3 you will need the 1Password 3 extensions. The version 4 extension you are attempting to install will not be compatible with your version of 1Password.

    Please download the version 3 extensions from here: https://agilebits.com/extensions/mac/index.html

    This should get you all sorted out, but if you hit any further snags, we're here for you :)

    Sorry to bother you again, but I used the link above and the loaded extension does not seem to work. It is not giving me an error message, but just does not do anything when clicked. In the Safari preferences it says it is 1P 4.1.0

    I'll admit, you have me a bit baffled here. I've tested that link and it does install the version 3 extension for me. I'd like to take a closer look at your system.
    I'd like to ask you to create a Diagnostics Report from your Mac. They are easy to generate and gives us an idea of what is going on 'under the hood' with your installation of 1Password. To create a diagnostics report on OS X 10.7 Lion or OS X 10.8 Mountain Lion, please follow these instructions:

    Then attach the entire file to an email to us: support+forum@ agilebits .com

    Please do not post your Diagnostics Report in the forums, but please do include a link to this thread in your email, along with your forum handle so that we can "connect the dots" when we see your Diagnostics Report in our inbox.
