Licensing Cost Questions
I'm trying to understand your cost structure. According to your license terms, the initial payment for Mac/Win license covers all minor version updates. So if I buy v5, I'm covered until version 6 is rolled out. However, it goes on to say that this only applies if purchased directly from AgileBits.
Question, is the above correct and how often do major version upgrades come out (annually, bi-annually)?
Moving on, it seems as though iCloud synching is only available if I purchase from the Apple App Store. So people who want iCloud must purchase from there which may restrict their upgrade availability.
Question, how are upgrades handled for the version purchased from the Apple App Store? Do I continue to get minor version upgrades?
Moving on, there is also an in-app purchase for $9.99.
Question, if I've already purchased the Mac license, does this fee still apply if I want those features? Is this a 1-time fee vs version based?

Please note that the in-app purchase is for the iOS version of 1PW. The program is sold per platform, so iOS and Mac versions are bought separately. There is a Mac/Windows bundle if bought direct from AgileBits, and they will organise a discount if you buy the Mac version from the App Store and then need Windows also.
Perhaps the wording is unclear, as minor updates are also free when 1PW is bought from the App Store. However App Store purchases are tied to your Apple ID, and an actual license is not used.
However (due to the way Apple organises things), when a paid upgrade comes out you can get an upgrade discount for a direct purchase, but have to pay the full price for a purchase from the App Store.
I'm not sure how often major upgrades have come out. But, though the upgrade from 3 to 4 was a paid upgrade, the one from 4 to 5 was free.
Yes, iCloud sync is only available in the App Store version (again due to Apple's restrictions, not to any decision by AgileBits). But many people, including most of AgileBits staff, are happy to use Dropbox sync, which is available on all versions.
Once you have paid, you can continue to use that version (and minor updates) as long as it still works on your OS. But you may have to pay a fee to make a major upgrade, and sometimes an OS upgrade (or a new OS on a new computer) may mean that the old version no longer works. That said, 1PW3 still works on Yosemite, though it does have some limitations.

Danco seems to have answered all of your questions pretty accurately. I just have a couple wee bits to add.
The in-app purchase of Pro Features within 1Password for iOS is completely optional. You can use the app for free with core features for as long as you want, and you'll have access to all of your saved items. But Pro Features make it easier to organize your items, as well as access multiple vaults and other advanced features.
And because 1Password licensed per person, per platform, you'd only have to make the purchase once to use Pro Features on all your iOS devices (if you have more than one). As long as you're signed in with the same Apple ID, you'll be able to restore the purchase from inside the app.
Likewise, a single purchase of 1Password 5 for Mac will allow you to use 1Password 5 (as well as versions 3 and 4) on all your Macs, as well as the Macs of up to 5 family members in your household. Ditto Windows.
As far as major upgrades to 1Password for Mac, we're on the fifth one in nine years. And as Danco said, "once you have paid, you can continue to use that version (and minor updates) as long as it still works on your OS," meaning licenses never expire, and you'll never be forced to upgrade; so, as long as you have an OS that supports it, you can use your licensed version of 1Password forever.
