Do I have to buy it again??
I have 1Password on my iPhone, but now I see it is also in the app store for my Mac. When I try to download it, it tries to charge me $50 again...Does my purchase to my iPhone not register here? Do I need to pay for this app again?

Licenses for 1Password are sold on a per user, per platform basis. Since iOS and OS X are separate platforms, this means that your iOS installation has nothing to do with enabling you to use the OS X version. If you want to use 1Passowrd on a Mac, that will require another purchase.
