Did the article from Stephen's reply help? I was able to find your license in our system and resent it to your email, so you can use a link in that email to apply your license file if necessary.
If you're still having trouble with that, please make sure you've installed any available updates for 1Password. Your message indicated you're using version 5.0.1 which was released almost a year ago. The latest version of the AgileBits Store version of 1Password for Mac is 5.3.2. From the main 1Password app, go to the menu for 1Password > Check for Updates.
