How can 1P handle a login that requires 3 elements instead of just a username and password?
I have a bank website that requires that I input a company number (for my company), then a username, then a password. That’s 3 elements instead of two. Login items in 1P only seem to allow for just a username and password.
1Password Version: 5.3.2
Extension Version: 4.4.3
OS Version: 10.11
Sync Type: DropBox

The usual solution for problems like this is to create and save the login manually. Have you tried that? (That allows 1P to "see" the structure of the page when creating the new login.)
