I'm repeatedly getting this error: 1Password failed to connect to 1Password mini.

I repeatedly get that error. I have uninstalled, reinstalled, prayed... nothing works.

I got the same, initial error message.
Upon looking at Safari's Extensions, I found there was a 1PW mini v4.4.4 update to be installed. Upon doing so and restarting Safari, 1PW 5.4 opened without problem.
I'm using 1PW 5.4 MAS.

Please see if this AgileBits post helps you. However, that's a pure hunch on my part as you've not told us exactly what version of 1P for Mac you are using.
