Since 1Password 5 was a free upgrade from version 4 and uses exactly the same license file to register as version 4 does, you will indeed be set to use version 5 when you are ready to do so. No additional charge will be required.
Where are you planning to buy? If from the Mac App Store, I'm not sure what will happen when you try to buy 1Password. But you should pursue buying there if you want iCloud for syncing. At least for now, Apple only permits applications they sell themselves to have access to the CloudKit protocols needed to sync with iCloud. If you buy from AgileBits, you'll need to use Dropbox for a cloud-based sync solution. Dropbox has some advantage that are absent with iCloud, such as the ability to sync multiple vaults and allowing you to share your data with other users. You will also get discounted upgrades at such time as a paid upgrade arrives if you buy from AgileBits. Apple doesn't allow discounted upgrade pricing. Obviously, it's your choice, but I want to be sure you know before you decide.

Hi @mrhash,
I hope hawkmoth's answer helped. Apple's Mac App Store is tricky as you're running Mountain Lion but what you want to achieve is definitely possibly if you purchase from our AgileBits Store. If you have any questions please do ask.
