I'm afraid it's impossible to help you without some basic information. Please tell us:
- What version of 1P for Mac you are using
- What version of 1Password for iOS you are using
- What sync method you use to sync your 1P data (Dropbox, iCloud or wifi)
(Use the links I've given for instructions on how to find the relevant version numbers.)

@edcarhart: Indeed, without some basic information it's hard to say what might be going wrong and how we might right it! Please tell me the OS, 1Password, and device versions you're using, the exact steps you're taking, and what is (or is not) happening the way you expect so we can figure out the best plan of action.
