If the email address you use for the forum is the same as the one you used for the purchase, your forum post is all that's needed. An AgileBits staff member will come along (though it may take time) and make the refund.
If you used a different email address, then sending an email to support+licences@agilebits.com with the details should work.

Indeed, I did notice two charges, made six minutes apart, for 1Password 5 for Mac licenses, registered to your email address. I have processed the refund for the second charge; you should see the funds in your account within a few business days.
Our apologies for the double charge! We'd love to know what you experienced in our store checkout; if the store is causing accidental double-charges, we'd definitely like to fix that. Did you notice anything unusual? Thanks in advance!
