No prompt to save changed password on website
So I just created a new password for Kayak.com. I used to have an old password there, but they required me to create a new one. I used the password generator built into 1password. It filled in the fields and automatically pressed accept. Yet it did not update its own record. I checked and the old password is still in 1password. What did I do wrong? is it not supposed to update its own record when I use it to create a new password for an existing login?

After you've filled the fields on the change password page and pressed the 'submit' button to continue, 1Password should pop up and prompt you to update your existing Login. If not, that new password will (by default) be copied to your clipboard so you can open up 1Password and manually update your Login item.
That new password is also saved into the Passwords category as a failsafe. You should still have access to it there.
We're working on ensuring that 1Password recognizes all change password forms, I'm sorry that it missed this one - I'll let our developers know that we could improve here. :)

Not sure why, but 1password did not prompt me to update the my existing login. It entered the password into the password field, and the confirm password field, then automatically pressed the accept button. It also did not copy the password to my clipboard, even though I have that option selected.
I ended up having to reset the password again. This time I manually copied the password. pasted it into the two fields, and clicked accept myself. This time 1password did prompt me to update my existing login and things are ok now. However I am reluctant to trust that 1password will work properly in the future as this is not the first time I have noticed this issue. However, this time, it bothered me enough to actually post about it.

Thanks for describing what happened! I'm sorry it didn't work the way it was supposed to, but I think I understand what happened.
The first thing I wanted to explain is that you won't actually lose your generated password if 1Password doesn't prompt you to update your Login. When you click the Fill (or Copy) button in the Password Generator in 1Password mini, that creates a new Password item in 1Password. That serves as a safety net for situations like this. Simply open the main 1Password app, click on the Passwords category on the left sidebar, and you should have a new item for the new password you generated (you can sort the list by Date Created if it helps). From there, you can copy/paste the new password into your Login item for that site. We have more information here: How can I recover an unsaved password?
Now, there's another issue here, which is that you weren't prompted by 1Password to update your Login item the first time you changed your password on that site. That's actually due to a specific bug in the current version of the 1Password extension, which prevents it from prompting you to update a Login item if you used the Fill button in the Password Generator. I'm very sorry for the inconvenience!
The bug only happens when clicking the Fill button though, which is why it worked when you copied & pasted the generated password into the 'change password' form on that site. That's actually the workaround we've been suggesting to customers. You can either highlight the password and copy it manually, or click the ▼ button next to Fill and choose Copy. When you paste it in the form and submit, 1Password should ask to update your Login item.
That bug has actually been fixed in the latest 1Password extension beta, and barring any unforeseen circumstances, the fix should be included in the next regular update for the extension. I don't have a timeframe for that, but in the meantime, if you'd like to install the beta extension, just follow these steps:
- Open the following link in your browser: https://agilebits.com/onepassword/extensions
- Click the link for Enable betas (underneath the Install button)
- Click the big green Install button and follow any additional steps
If you don't want to install the beta extension, just remember to use the copy & paste workaround for now, if you need to change another website password.
