I have done the steps of removing old 1password app and still have this problem it used to work when I first upgraded.

- Your OS X version number
If you're using 1P5 on OS X El Capitan you could try the suggestion in this AgileBits post.

@Flutetears: Definitely restart your Mac, and double-check that you don't have any additional copies of 1Password. OS X will often launch the wrong copy of mini if any exist.

@Brenty I had done just that before posting I did have a old version and deleted that but not go
