QR code detection failed - 1Password for Mac
1P doesn't show in Security.prefPane > Privacy > Camera, but it's in Accessibility & Screen Recording. What do I need to tweak?

Does the problematic QR code have a bright background or is it dimmed? There have been a few sites that pose problems. Is the site one you can share?
