This happens if you have both the website (or beta) version of 1Password 4 running alongside the 1Password 4 Mac App Store version.
If you want to keep both versions of 1Password 4 installed on your Mac, then the solution is to disable 1Password mini from the version you're not using. From the 1Password app, go to 1Password > Preferences > General, and disable the option for "Always keep 1Password mini running".
If you don't need to have both versions of 1Password 4, then the solution is to delete the version you don't use and empty the trash, then restart your Mac. (Note: Do not use any cleaning tools to remove excessive files from 1Password, you can lose your backups and data file like this.)
That should take care of the extra 1Password mini, but let us know if you need more help with that.

I have the same problem (two 1Password4 minis in menubar). I would like to delete the website/beta version of 1Password4 and keep the AppStore version. How do I tell which is which, and how can I delete the correct one?

I'm sorry you're having trouble getting rid of the extra 1P mini/helper process from the website/beta version. Thanks for mentioning you prefer keeping the MAS version; that should make fixing this quicker. :)
Please give this a try:
- Quit 1Password 4 and web browsers that have the 1P4 extension installed.
Run these four commands in a Terminal window:
launchctl stop 2BUA8C4S2C.com.agilebits.onepassword4-helper launchctl remove 2BUA8C4S2C.com.agilebits.onepassword4-helper defaults write com.agilebits.onepassword4 KeepHelperRunning 0 defaults write com.agilebits.onepassword4 ShowStatusItem 0
Restart your system.
- Relaunch 1Password 4 and run Troubleshooting > Restart 1Password mini from the Help menu.
If an extra 1P mini is still hanging around after that please run this command in a Terminal window and post its output:
launchctl list | grep agile

Hi Oversoul. Unfortunately, the second 1P mini returned shortly after it disappeared. I ran the four commands again, and this time they didn't get rid of the second 1P mini key. So, I ran the fifth command you suggested. Here is the output:

I'm sorry that didn't fully work for you. This might do it:
- Open Terminal.app (from the /Applications/Utilities folder).
Copy and paste this single command into the Terminal window:
defaults write com.agilebits.onepassword4 ShowStatusItem 0
Close the Terminal window.
- Empty Trash.
- Restart your Mac.

Thanks. I tried that, but it didn't work. I still have two 1P Mini "key" icons. I just emailed my diagnostics report.
