1P3 used a licence number. 1P4 and 1P5 use a licence file. What version are you trying to licence and from where did you get the licence?
Note that the upgrade from 1P3 to 1P4 was not free: you had to pay for it.

I have 1Password installed on 4 systems. The one I am currently on is OS X 10.10.1, 1 Password 5 Version 5.0.2.
A former employer bought the license for me and I have the email she received from Agile Bits, and forwarded to me, with the license information. It is a couple years old.

That then is almost certainly a licence for 1P3 which won't work to licence 1P4 or 1P5 (1P4 and 1P5 both use the same licence, but not the same licence as 1P3).
If you are running 1P5 downloaded direct from the AgileBits store (as opposed to the Mac App Store) I'm afraid you're soon going to get the "trial expired" message referred to in this sticky at the top of the forum. If you then want to continue using 1P you're going to have to buy a licence.

@shabbelle - That license date would be eligible for a free update to version 4 and hence also version 5. I don't know how it complicates matters that your license was purchased for you by someone else, but I think you can get things straightened out if you send all the relevant information you have, including your name and address (in case your employer bought it in your name), as well as the name of that employer and the email address s/he used to make the purchase to support+licenses @ agile bits.com. The more information you can provide, the better. I'm not sure if it will help, but adding the license key your have for version 3 might help too. Don't post all that personal stuff to the forum.

Actually, now that I look at the email, it says at the top "1Password 4 for Mac". So perhaps I do not have to make a license purchase at this time? I will try emailing my info to support+licenses@agilebits.com. Thank you.

If you buy 1P5 from the AgileBits store these are the licence terms (taken from this page):
Our software is licensed per person and per platform, not per device. Each user and each platform requires a license. Licenses can be shared by up to six family members living in the same household. Licenses can not be transfered or re-sold.
If you buy from the Mac App Store (which you would need to do if you wanted to use iCloud to sync 1P data between your Mac and your phone) you can run 1P5 on any Mac that shares the same Apple ID or is part of a family sharing arrangement. Note that the AgileBits store version of 1P5 does not support iCloud sync of 1P data following changes made by Apple on the launch of OS X Yosemite. Both versions support sync of 1P data by Dropbox or local wifi. If you buy from the Mac App Store you won't be able to obtain any discount on the price you might obtain when buying direct from AgileBits.
1P for Mac and 1P for iOS are different products so you'd need to download 1P for iOS from the App Store on your phone (not from the Mac App Store). It's free but there's an in-app purchase to enable pro features.
