I deleted the 1Password5 from my MAC but don't see an option in my history to redownload.
I have a receipt from purchasing 1Password in iTunes, but I just recently received a trial over notice when I've been using this app since 09/05/14. I don't see where to re-download without paying for this again. And I consistently have sync issues.

I see you have read the knowledge base article about the "trial expired" message. Did nothing in it help at all?
I'm a little confused about your reference to:
purchasing 1Password in iTunes
You would have bought a licence for 1P4/1P5 from the AgileBits store or you would have paid and downloaded from the Mac App Store. Please tell us exactly what version of 1P for Mac you are using.
Note that if you need to retrieve your licence from AgileBits (assuming you bought from the AgileBits store) you can do so on this page by giving the email address you used when you bought your 1P4/1P5 licence.

Just to add to Stephen_C's comments, if you bought 1Password for Mac from the Mac App Store, you'll need to be logged in with the same Apple ID that you purchased it with, and then you should see it in the "Purchased" tab of the Mac App Store, with the option to install it from there.
1Password is licensed per user, per platform, so if you've only purchased Pro Features for 1Password for iOS, this purchase does not apply to the Mac app.
