I'm sorry that you're seeing an unexpected "Save Login" prompt when using Bank of America's website. This is a known issue that our development team is tracking and hopes to fix in the future. For the time being, to workaround the issue, turn off auto-submit for that specific website:
- Click on the 1Password icon in your browser's toolbar.
- Select your Login item for Bank of America.
- Click the three dots in the upper right corner of the item and then click Don't sign in automatically.

1Password for Safari should unlock when you unlock the desktop app. If that's not happening then it sounds like the communication between the desktop app and the Safari extension isn't working. Try this:
- Open and unlock the 1Password for Mac desktop app.
- Click on 1Password next to the in the menu bar.
- Click Settings.
- Click Browser.
- Make sure that "Connect with 1Password in the browser" is on.
Then in the browser:
- Open your browser.
- Right-click on the 1Password icon in your browser's toolbar and click Settings.
- Make sure that "Integrate this extension with the 1Password desktop app" is on.
If that still doesn't work then do you see a green dot in the browser settings page? Or a different coloured dot and an error message?

Thanks for the BofA workaround. Unfortunately what I'm seeing doesn't match your instructions:
There is no "Don't sign in automatically" option (screenshot has heavy redaction). I also don't see this anywhere, and I don't remember this being a feature of 1Password, although it would be nice.
If you edit the login item, there are options next to the website related to auto-fill, but that will only stop it from even being presented for the site, so I would have to go out to the app or quick-access to copy the password and paste it, so it's the same as deleting the website in effect while keeping it as a reference in the app.
RE: Extension auto-unlock on app-unlock
Yes, I've reported this problem many times, and these same steps to reconnect the extension with the app are offered every time, but they rarely work, and if they do, it doesn't last long.
The app reports it is connected (green dot) in every case that I experience having to manually unlock the extension by clicking on it.
I have 27+ 1Password accounts, and this issue seems to be related to that. I reduced the number of signed-in accounts to my 10 most accessed, and that has helped with the "Reload 1Password" extension issue but does not help with the extension unlocking on app-unlock extension issue.
