Autofill not working for www.sbcdonor.org
I can't get 1Password to fill in either the username or password on the login page (www.sbcdonor.org/index.cfg?group=login). It does work on the mobile site (https://www.sbcdonor.org/mobile/index.cfm?group=account&function=signin). Tried looking at the HTML source of the pages and could see that the field definitions differ between the two, but I really don't know enough about how 1Password works to figure out what might be going on.

I couldn't see the login page by using your first link (it gives me a 404 not found) but have you tried re-creating and saving the login manually for that page?
The linked page includes the following:
Saving a new Login like this can be useful for Logins that are either problematic to begin with or were once working but have since stopped. It allows 1Password to refresh everything it “knows” about the page. Login pages can change as websites are updated, and this can be a very helpful tip in those cases.
That's why the procedure might help in this case.

Sorry, there's a typo in the first URL. Should be https://www.sbcdonor.org/index.cfm?group=login
I tried saving a new login (entering my credentials without submitting, etc.), but 1Password still won't fill in the fields when I go back to the login screen.

Using your original Login item for the Donor Login - Stanford Blood Center page, or a manually saved new one like @Stephen_C suggested, should fill okay there with Firefox. However, 1Password filling isn't cooperating there with Safari or Chrome so I've filed a bug report for that. Thank you for reporting this issue!
