How can I get version 5 on my MacBook?
I'm unclear whether you mean physically or in terms of getting a licence.
The first point is that you'll need to be running OS X Yosemite to run 1P5 on your MacBook. If you're already doing that you can (depending where you bought the original 1P5):
- download from the Mac App Store to the MacBook using the same Apple ID as when you purchased 1P5 originally; or
- (if you bought from the AgileBits store) download from the AgileBits store and copy your existing licence from the iMac to the MacBook.
If you're actually asking some other question and I've misunderstood please post again.

i am having a similar issue in that I need to install a completely fresh version 5 onto my MacBook Pro. I have version 5.1 on my iMac, and I have the iOS versions on my iPad and iPhone. On my MacBook, I went to the Apple App store and searched 1Password. When the page came up, instead of "Get" it has a button with "$49.99" on it. I thought the software license was good across more than one computer.

Found out how to do it. Looked at another post, and they suggested looking under purchases in the App Store. When I looked in the App Store under purchases, it turns out I was not signed into the store. After I signed in, the app was available under purchased items. Hope that helps.

Glad to hear you managed to resolve your issue @Wyzer, I can imagine it's a little unsettling to be informed you need to spend $49.99 if you know you've already purchased it.
@pegasso4444, how did you get along based on Stephen_C's post?
