Again, updating 1Password doesn't update the extension in Safari. I have since updated the extension and it seems to have solved the issue.
1Password for Safari is a separate app from the 1Password desktop app and will update separately. However, you can set the Mac App Store to automatically update 1Password for Safari:
You can also give the Mac App Store the ability to send you notifications when an update is available: Change Notifications settings on Mac - Apple Support (CA)
